Recently, the number of cesarean sections performed has increased significantly. This, in turn, contributes to the appearance of pregnancies with an abnormal location in the cesarean section scar. Is it possible to deliver a pregnancy in a scar? What are its complications?

Pregnancy in scar-what it is

Scar pregnancy is considered the rarest form of ectopic pregnancy. Pregnancy in a cesarean section scar is a consequence of a previously performed cesarean section. It can, which is less common, also be a pregnancy in the scar from uterine procedures the woman has undergone. This occurs as a result of improper implantation into the musculature. This may be related to, among other things. With abnormal vascularization of the scar and its hypoxia. Some researchers believe that the pregnancy develops in the scar due to the loss of wall thickness, the so-called “scar”. niche. It is recognized that scar pregnancy is more common in patients who have undergone a uterine cavity curettage, IVF procedure, ectopic pregnancy or an associated placental pathology that occurred in the past.

Pregnancy in twins- symptoms

Pregnancy in a scar can give symptoms the same as a pregnancy with a normal location, however, it can also be asymptomatic. Sometimes, however, it is accompanied by bleeding from the genital tract or lower abdominal pain. In some women, the first sign of pregnancy in a scar are symptoms associated with hypovolemic shock-acceleration of the heart rate (tachycardia) or pallor of the skin and severe pain.

Pregnancy in scar- management

The diagnosis of pregnancy in the scar should be made by an experienced ultrasound gynecologist. Sometimes, especially in advanced pregnancy, an MRI is necessary. Most often, due to the risk of serious consequences of maintaining such a pregnancy, it is necessary to terminate it. Management can be pharmacological or surgical. Methotrexate and potassium chloride are used for this purpose. Pregnancies in the scar can also be removed by laparotomy. Most often, the procedure or the use of pharmacotherapy should take place in early pregnancy. As a result, it is possible to preserve the uterus and develop another already properly located pregnancy and give birth to a full-term baby. The older the pregnancy, the greater the risk of ingrowth in the bladder or surrounding tissues. In the case of delivery by cesarean section, it is not uncommon for doctors to have to remove the entire uterus while supplementing the patient’s significant blood loss.

Pregnancy in a scar- complications

Pregnancy in a cesarean section scar carries possible serious complications. These include. the development of an abnormally positioned placenta (such as a placenta previa), miscarriage, and in later weeks, the risk of hemorrhage and loss of the uterus. A pregnancy in a scar is considered a life-threatening pregnancy for the woman. This is because, in addition to hemorrhaging, the uterus may rupture.
